When you make an appointment, our staff may ask you for your insurance information to help verify your eligiblity and inform you of your benefits. Please also bring your complete insurance information to the initial visit.

As a courtesy, we will process any insurance claims for ongoing treatment. We will attempt to give you an accurate estimate of your portion of the insurance co-payment. Our office is committed to helping you maximize your insurance benefits as allowed by law.
